Patient safety
Report a side effect
Tell us about any unwanted effect from a Draxer product. Reports from patients, carers and healthcare professionals all help us keep our medicines safe. You do not need to be certain that the product caused it.
If someone is seriously unwell, get medical help first.
Go to the nearest hospital or contact the prescriber. Report to us once it is safe to do so. This form is not monitored around the clock.
Who can report
Anyone. The person who took the product, a family member or carer, or a pharmacist, doctor, nurse or other health worker.
What to have ready
The product pack, so you can give the batch number and expiry date. The date the product was started and the date the effect began. Any other medicines or supplements being taken.
What happens next
A Draxer pharmacist reviews every report. We acknowledge it within one working day and may contact you for more detail. Serious reports are passed to the Food and Drugs Authority, Ghana, as the law requires. Your personal details are not shared publicly.
Side effect report form
Four short sections. Fields marked * are required; everything else helps our pharmacists but can be left blank. It takes about five minutes.
